Lung HIV Common Forms/Common Data Elements

This section contains the Lung HIV Common Forms that include the common data elements collected by the individual clinical sites and a diagram listing the common data elements collected that comprise the Lung HIV common database. Participating clinical sites in the Lung HIV study use either these common forms or site-specific forms that includes the common data elements. Current and previous revisions are included in this section.

Annotated copies with variables names are included for the demographic, diagnosis and pulmonary function test forms. The questionnaire forms were produced as templates for common data elements. All sites use site specific questionnaires which include most of the common data elements from these templates.
